I've been meaning to get a personal pathtag but just don't know who to contact and what I would need to provide.
I've heard the name Kris Mackey. Is that who I should use?
Would I send the creator a picture of what I'm wanting?

SnowySwan and I used pathtags.com and worked with one of the listed designers on the website. We sent her pictures and collaborated to the finished design that we then submitted to pathtags.com

Piling onto Cameron's post, I recommend going to the Pathtag.com website Design Page and reading all about the process. Then, under the "Get Going" section, there is a link to Pathtags.com's Design Partners, who are people that specialize in pathtag designing for a very nominal fee (some will do it for just a couple of the pathtags after they are produced).
Chris Mackey is listed in the Design Partner's section as AURA Design Group, fox-and-the-hound.
Design Partners on the list that have designed GeocacheAlaska! pathtags include negativesleep, WeBBleR, and Mickey and Goofey.

Fuzzy, I worked with Nicole Florian at "Pathtags Art" for our pathtag. She is listed at pathtags.com under Official Design Partners.

I can't find cost listed anywhere. How much does it cost to design and purchase tags?

www.pathtags.com/community/pricecalc.php is accessed via the "Buy Tags" button at the top of the pathtags.com home page and clicking on the "Starter Kit Price Calculator." Clicking on "Read More" under "Design a Tag" provides access to a jpg template and the detailed specs for use with any graphics program, with specific instructions for the file type to save if using Adobe Illustrator. A good Pantone color chart for specifying exact colors is located here. Be aware that one's monitor "tuning" and variations in color printer output will affect how colors appear relative to the "true" colors the manufacturer will use when creating the pathtag.
